Governance and Documentation Must Be Unified

In autonomous environments, governance cannot exist separately from documentation. When policies, workflows, evidence, and reviews live in disconnected tools or formats, accountability fragments. Decisions may be made, but the reasoning behind them becomes difficult to trace and explain.

Effective governance requires a continuous internal link from policy to outcome: policy informs workflows, workflows generate documentation, documentation enables review, and review shapes future decisions.
